Dear Visitors,At the Chatham County Safety Net Planning Council, our partners are constantly looking for ways to serve you better with added services and extended hours as opportunities become available. Below you will see a summary of the most recent changes and progress:
- The Chatham County Safety Net Planning Council is proud to announce we were awarded the 2010 Joseph D. Greene Community Service Award by Healthcare Georgia Foundation. Representatives from CCSNPC accepted the award at the Connections 2010 conference in Atlanta, Georgia on March 31, 2010. Healthcare Georgia Foundation's Joseph D. Greene Community Service Awards honor extraordinary individuals and nonprofit organizations that are committed to improving the health and health care of the populations they serve.
- On April 14, 2010, the Chatham County Safety Net Planning Council launched our Health Information Exchange Pilot Project between Memorial Health University Medical Center Emergency Department and J. C. Lewis Primary Healthcare Center as part of the celebration of National County Government Month. The first of its kind in Georgia, our Health Information Exchange is designed to make access to care easier, by understanding that people do not have the luxury of planning ahead for a health crisis and may need to seek care at the last minute wherever they can find it. Wherever patients present for care, their records will follow them. In this way, the care they receive will be better, because clinical decisions made by their healthcare providers are based on complete information.
