Chatham Creating Access Now (Chatham CAN)
Through funding made available by a grant from Healthcare Georgia Foundation, Chatham CAN works closely with Council partners to develop a system focused on solving the long standing problem of coordination of services and access to specialty care for uninsured patients served by Chatham County Safety Net Planning Council (CCSNPC) health care providers.Through shared decision making and utilization of electronic software accessible by key CCSNPC providers, Chatham CAN will create the infrastructure to support, improve and track access to specialty medical care. A committee composed of targeted council members will identify the requirements of the technology and processes to assure accountability, measurement of outcomes and service delivery, patient privacy and ease of use of the system developed. On behalf of the Council, the Project Manager will work closely with physician leadership in the local Georgia Medical Society, large physician groups, hospitals and small practices to recruit and enroll volunteer physicians from the broad array of specialties vital to the success of this project.

Abercrombie Bio:
Debra Abercrombie, BA, a graduate of the University of Georgia, holds a Graduate Certificate in Public Health Policy and is an MPH candidate in Policy and Management at Emory University's Rollins School of Public Health where Abercrombie was a Georgia Preparedness Scholar. Abercrombie is a past recipient of the Georgia Public Health Association's Sellers-McCroan award and is a past chairperson of the Association's Epidemiology Section.
Ms. Abercrombie worked 13 years in the Division of Mental Health, Developmental Disabilities and Addictive Diseases before beginning her work in the Division of Public Health in 1990. Abercrombie served as Epidemiology Director for Northwest Georgia Public Health from 1994 until her retirement in 2009 after 32 years of service to the Georgia Department of Human Resources.
