ChathamHealthLink Architecture
ChathamHealthLink (HIE) maintains a complete server system hosted independently of the Provider Participants and managed by the Chatham County Safety Net Planning Council.The HIE utilizes OrionHealth's HIE solution consisting of the Rhapsody Integration Engine, which receives and processes patient data from the participants' electronic medical records and readies it for central storage.
In order to properly collate a patient's records across all participating health care providers, the OrionHealth system includes an embedded Catalyst Enterprise Master Patient Index developed by Initiate, an IBM company.
Once the submitted data is matched to the proper patient record and processed into a common format, it is sent to the Clinical Data Repository housed in an Oracle database.
Currently, the data set Chatham County Safety Net Planning Council provider participants contribute to the HIE includes:
- demographics
- allergies and alerts
- medications
- encounter summary
- medical history
- lab orders and results
The HIE's consolidated patient information may be accessed by authorized users at any of the participant locations through a simple, intuitive user interface called Concerto Physicians' Portal, another OrionHealth product.
Concerto also provides capabilities for usage monitoring and audits; patient opt out, and manual merge/un-merge of any records which have not have been handled by the automated EMPI.
ChathamHealthLink trains key users at each of the participants and maintains the user roles and access permissions of all clinical and administrative users authorized to access the HIE.
ChathamHealthLink standing policy and procedure committees guide the HIE and its participants in the implementation of best practices for reliable and secure operation of an HIE including federal, state and local laws and regulations related to the privacy and security of patient information.
