Through funding made available by a grant from Healthcare Georgia Foundation, Chatham CAN worked closely with Council partners to develop a system focused on solving the long standing problem of coordination of services and access to specialty care for uninsured patients served by Chatham County Safety Net Planning Council (CCSNPC) health care providers between February 1, 2009 and January 30, 2011.
Health Information Exchange Pilot Project
The members of the Chatham County Safety Net Planning Council (CCSNPC) have a vision that Chatham County's health care delivery structure will develop a fully integrated health information exchange (HIE) that improves patient health and improves the quality of care for all health care consumers.
ACT: Assess-Connect-Teach
Funded by the Southeast Georgia Cancer Alliance (SEGCA), ACT will provide Colorectal Cancer risk assessments and health education and Colorectal Cancer screening to uninsured adults in Southeast Georgia.
ACT II: Assess-Connect-Teach II
Funded by the Southeast Georgia Cancer Alliance (SEGCA), ACT II will provide Year 2 of Colorectal Cancer risk assessments and health education and Colorectal Cancer screening to uninsured adults in Southeast Georgia.
Georgia Health Foundation (GHF)
GHF has funded a pilot project to identify a specific population without insurance and without a medical home, to screen patients in a uniform manner and educate entire families about eligibility for health insurance and other health related resources.
PACT: Prostate Assess-Connect-Treat
Funded by the Southeast Georgia Cancer Alliance (SEGCA), PACT will provide linkage to medical homes and access to prostate biopsies for uninsured men in Chatham County who have positive screening for prostate cancer.
