Chief Medical Officer, Chatham County Health Department
Dr. Diane Weems has served as Chair of the Chatham County Safety Net Planning Council since its inception in 2004. Dr. Weems is also the Chief Medical Officer for the Coastal Health District, an eight county public health region spanning the coast of Georgia, Administrator for the Chatham County Health Department, and a Board Certified Pediatrician. As a representative of public health, Dr. Weems is viewed as a neutral convener of the Council.
Paula D. Reynolds, MD, MPH
Executive Director, Chatham County Safety Net Planning Council (CCSNPC)
Dr. Paula Reynolds joined the CCSNPC as the first Executive Director in December of 2008. She is an adjunct professor at Armstrong Atlantic State University in the Health Sciences department and has served as Associate Director of Pediatric Research at Memorial Health University Medical Center and as Executive Director of the Savannah Area Behavioral Health Collaborative.

The Executive Committee of the CCSNPC is made up of the Council officers- the Chair, Vice-Chair, Secretary and Treasurer and the Chairs of the Policy and Evaluation Committees.
