PACT: Prostate Assess-Connect-Treat
Funded by the Southeast Georgia Cancer Alliance (SEGCA) from July 1, 2011- June 30, 2012, Prostate Assess-Connect -Treat (PACT) will provide prostate biopsies to uninsured men with confirmed positive prostate cancer screenings. A care navigator will follow the patients through the entire care continuum, provide linkage to all recommended health and social services and insure that the patients are fully informed about prostate cancer and any proposed treatment options.Currently, numerous community agencies provide prostate cancer screening, outreach and education including the American Cancer Society, Community Cardiovascular Council, St. Joseph's Candler Hospital Systems, Memorial University Medical Center and a number of community groups. However, there is no organized, reliable process by which uninsured men who are found to have a positive screening can be referred to and ensured of a prostate biopsy if one is recommended by their health care provider.
PACT will operate in a system which closely mirrors the Navigation and Screening Model established by CCSNPC's previous colorectal cancer screening projects, ACT and ACT II. All of the components of this model are already in place and functioning in Chatham County and the surrounding region.
