To make an appointment:
Please call and schedule an appointment to be seen. For new patients, the wait time for an initial evaluation is approximately 1-3 weeks depending on the location seen. Walk-Ins are accepted (except at Savannah State University). However, patients with appointments are seen first.
What you need to register as a patient:
- Current state ID and social security card, passport, or birth certificate
- Verification of current address such as: Driver’s License, or Utility Bill
- Verification of Insurance such as: Medicaid, Medicare, or Private Insurance
- Proof of household income such as: Earnings, Social Security, Child Support, Unemployment and/or Worker’s Compensation
Discounts on services are available to qualifying patients. Eligibility is based on family income and family size.
